Fairfax High School Crew


		All of the following forms must be completed and returned to the Membership Chair with a check for the rower's fees in order for registration to be complete. Rowers with incomplete registrations will not be permitted to participate in Crew activities. These forms are available in either Adobe's Portable Document Format (PDF) or Microsoft Word Format . 		1. Application to Row and Join the FHS Crew Club		This form must be completed. Parents/Guardians must give written permission for their student to row. As members of the crew team both students and their families also pledge to volunteer for fund raising, regatta jobs, and other Crew Club activities.
		2a. Virginia HS League Athletic Participation & Physical Exam Form (4 pages)		This form is required for athletic eligibility. Every rower must have an Athletic Participation form on file for the current academic school year. If you already have a current form on file, complete the form below.
		2b. Medical Form on-file		If your rower’s current Athletic Participation form is already on file for the current school year, please return this form instead.
		3. Emergency Care Information		This form is kept on file by the coaches at the boathouse and accompanies the rower should medical attention be needed. All line items must be completed. Attach additional sheets of health or emergency information, if needed.
		US Rowing Release & Waiver Form		Signed form is required for all rowers who participate in regattas sponsored by the NCASRA and US Rowing.
		Photo Release Form		We may have opportunities and requests for photos of the Fairfax rowers. Please read and sign the permission form to include your student in promotional or fundraising photographs.
